examined by me on August 15

PRESENT' ILLNESS
The following is an account of an accident which occurred on
teaber 25, 1966 at 8 A. M.

hospital Corona Cormunity Hospital I Tva S treated by Dr _
Richard Nelsone He had epplied some stitches to what he
claimed was ezcessive bleeding He applied about three to
the lower side of Tly chin , end I don t know how mlany in the
left eyc. They took 3 series of x-rays ; and at that tine
I Tas not fully conscious I starled to cone to , when I felt
the coldness of the platfort

I realized something was wrong with my eyes et the moment I
started to come to When he started to insert the stitching

PRESENT ILLNESS cont'd:
needle in Iy eye and I more or less 1as much against that.
I didn t like the stitch to be there- He insisted that
it was necessary_ I didn t know the gravity of the wound
but I thought it wasn't necessary to be stitched_ I cou ldn 't
'see myself and he just told ne it was necessary_ He wanted
to keep me in the hospital fox a 12ek , but I didn't like the
I idea_ I did s tay overnight_
As the history will indicate_ the patient rambled a. bit.
I asked the patient what he kner about sand being in his eyes
after the accident and this was his reply _
"Dr , Nelson- when he the stitches in my . eye I couldn t
open the eye to see anybody due to the sand in ny eye I
had to tell Dr , Nelson to rerove the sand

but he told one
"1 of the nurses afterwards and shle took care of that.
With regard to the subsequent treatment received,_ the patient
Tas a little vague. Hav reviewed the medical file, it was
apparent that the patient had been treated a short period
of time and discharged by Dr Nelson

and he then returned
to Dr

Nelson with comp laints , end the latter referred him to
an eye, ear, nose and throat specialist, Dr Paul Nilsson.
When I asked the patient 2bout this point , this is what he said_
"They were trying to arrange that appointment date;
between Dx , Nelson and Dr Nilsson, then we had to 0 c k. that
through the office which took about two weeks 01 SO It Was
a matter 0f trying to an agreenent Dr Nelson pparently
considered [le discharged because when I went dowen to See him
again he instructed his secretary to contact the conpany to
reopen the account he did not ect 0x treat ne after that:

The patient further stated;

Miller He gave m2 tests and didn't do anything
as far 2s treatnent . I was uider his care three or foux'
visits About 3 month later he said I should sce Dr Kiehn,
because I told him I had moved from Corona and back to Iy
original address _
First of 2l1 he (Dr _

Kiehn) asked if there were any broken
bones in this region. I told him I did not know _ He ordered
x-rays taken. He gave Ie 2 small tube of some lubricant and
that Was a sample type, and I used and it ren out within
a : week = It' didn't seem to help much_

He didn t say anything

I al80 asked the patient ebout the recomendation of Dr
Kiehn that he be seen by a neurosurgeon named Dr _ Robert
Fiskcin_ The patient sumarized this situation in the following
way :
Ihat was a very abortive attempt by Dr Kiehn _ I waited
three months after Dr Kiehn told me . It hasn t come, I
never saw the neurosurgeon. I never received the notice fron
the insurance company He (Dr _ Kiehn) hasn't discharged
he said until I see the neurosurgeon , and 28 yet: I have not
" seen hin, SO hor can I go back to himo
With regard to the patient " S present symptoms

he alleges the
following:
much facial disconfort The eye I can 't rotate
too tighto I can't look in both directionz 2s I used to ,
depending on the position of nly head _ I can 't shift the
gaze back and forth. I never did complain about the vision.
I seen to fail the side vision_ M1
The patient further indicated that there hac beein no rovement
in his condition since his accident.

REVIEW OF MEDICAL FILE cont'd:
The report of Dr. Kiehn is in general negative, however
M he conments on an "inconstent' constriction of the visual
field and in addition refers to 2 fibrotic band in the left
upper Thich he feels maybe the cause 0f the patient' s
ill defined symp toms

He further intinates that this condition
might require surgery
There are no other ophthalmological reports of significance
in the file_
EXAMINATION
Vision 20/20 Near: (Right eye)
20/20 J-2. (Left
External Structures: There is no apparent scarring of either
eyelid_ The ocular adnexae and globes
are
grossly negative.
Extra Ocular @uscles Grossly intact No diplopia demonstrated_
Pupils: Round regular and equal with normal reactions _
Tactile Tensions: Both eyes: Not elevated
Fundi: The were dilated; the fundi were studied with
both direct and indirect ophthalmoscopy They Were
found to be consisient with the stated (23)

EXAMINATION cont d:
Visual Field Screening cont d:
The examination was then repeated at the
tangent screen using both the 4/1000 &nd
2/1000 white targets This denons trated
marked constriction of both visual fields,
but more So in the left eye than in the
right It should b2 noted that the amount of
constriction With the two different targets
was not proportional to the size of the terget
used, The examination was then concluded using
the standard Perimeter_ This test shored moderate
constriction of the right field and marked con-
striction of the left field When the various
fields are compared , it is obvious that: the results
are highly inconsistent and in n0 way could b2
related to any organic process involving either
the eyes or the intra-cranial visual pathways

Stereopsis: Patient has normal binocular function

DIAGNOSIS
Essentially.normal eye examination_
COMMENTS
Based 01 ny examination, the history obtained _ and the
records presently &vailable, I do not believe that this patient
sustained any peraanent disability

medical exanination is not demonstrable at the present tine
and 1 strongly 2dvise against any atterpt to resort to surgical
intervention _ This patient, in my opinion_

utilizing any therapy of this nature Namely he has symp toms

which are not organic in 'origin_

and in addition has no
proof whatsoever that any foreign material is retained in
the left orbital area 0
With regards to the symptoms alleged by this patient,_ there
is no thing whatsoever in the patient ' S exanination to sub_
stantiate 2 claim of an orgenic injury_ As noted &bove,
nearly all the subjective tests of visual function have
clearly demonstrated a pattern of inconsistency which negates
the possibility of any injary to the eyes or the intra-cranial
visual pathweys _
CONCLUSIONS
As 3 result of the above reported injury this patient did not
sustain a
Permanent disability involving his eyes _ No further
medical trea tment is indicated as _
the patient S condition is
not industrially related

He fell and sustained injuries_ He was taken to
the Corona Community Hospital He had . a laceration
of the left eyelid_ a laceration of the chin con-
tusion of the and of the left hand _ The laceration
were sutured_ He states that at the time of the fall
he was briefly unconsciou: He was under the care
of Doctor Nelson He. was dischar the next
and remained under further care by Doctor Nelson
and hewas subsequently referred to Doctor Miller
and then to Doctor Kiehn He states he still is
under the care of Doctor Kiehn with treatments to
the left he is given some eyedrops During
this time

COMPLAINTS : The patient s tates that he
still has cls iderable discofort
in the Spine particul_ in the low back but
some discomfort also along the upper He
states that this discomfort occurs whenever he
moves bends s toops or stands for a long time
He does not know if the discomfort is increased
by coughing or sneezing beceuse he states he never
coughs or sneezes Discomfort is greater in the
morning but at times or in cold weather also at
night _ He has n 0 radiated symp toms to the lower
extremities He complains about the left eye where
he states that the skin on the inner s ide of the
eye is tight and uncomfortable especially in colder
weather He states that this my face off
balance" He indicates that the scar on thechin
region is indurated and gives him a feeling of
pressure He states that at this time he has' no
impairment of vision_
PAST HITORY: With above employer for about
six months He states that never
in the Past did he have any industrial claim, signi-
ficant injury, surgery or significant sickness
EXAMINATION: Age 23 height 5 ' 44" weight 115

minimally indurated There are no reactions of
or tenderness no suggestion of regional disability
from this scar
Upper extremities He states that he had a contusion
to the left hand but he states- that symp toms have sub-
sided entirely: On regional examination there are no
ful reactions no limitations of

is normal he wears no support he walks on his toes
and heels and he

comfort is at the lumbosacral level there are no
points Qf tenderness and there is

flexion is displayed by the patient but on sitting

from the Good Samaritan Hospital
and are films of the skull and of the lumbar
The findings are
entirely normal and negative _ The re
are no signs of recent or old injuries no
abnormalities of the disc spaces no S
ignificant
arthritic changes There 18 postural swayback.
SUBMITTED FILES These are records of Doctor Nelson_

Doctor Yamshon_ The records

complaints and uries incident to the accident of
September 25

Examination by several eye
specialists is negative for
any signs of an injury
to the left eye or for any signs of disability
attributable to the minor scar near the left; eye .
Examination by neurosurgeons i8 reported to be
negative with reference to neurosurgical findings .
A report by Doctor Yamshon refers to subjective
factors of permanent disability involving the low
back and to scarring and disfigurement incident to
the scars I thoroughly disagree with these con-
clusions
CONCLUSION : With reference to the scars and
to the patient' s low back, it is
my
opinion_ that this patient S
subjective complaints
are entirely unsupported by regional evaluation of
the findings _ Without his assistance, I would not
have noticed the scars their cosmetic defect
is therefore
)
nil I also do not belige that
these scars account for any subjective discomfort
of or tenderness There was an apparent minor
injury to the left hand fromivhich the patient has
ectively and objectively recovered He clains
persistent symptoms involving the low back where
regional examination is not suppor even to a
minimal extent these complaints _ I conc lud e that
in my
opinion he has
fully recovered

from a sub-

injuries with objective resicluals consisting ofonly
of minor scars Tvhich do not account for any disability
or cosmetic defect. Based on the submitted records

Mr . Sirhan came to this office on 'February 1967 having been
referred Eo me by Dr , Milton A. Miller 0f Otarioe The history of the
case as I receiwed it from the patient i5 briefly as follows;
The. patient exercised the horses at the Granja Vista Del Rio ranch
In Corona. On September 24 , 1966 , he waS throin from his horsa and suffered
injuries around the left eye He thinks he Wis unconscious for a brief
time He was seen at the Corona hospital by a Dr Richard Ne lson end the
vounds around the eye were sutured. In addition to the wounnd around the
eye he had a wound under the chine This WJs also sutured, Four days
later the sutures wcr€ removed He suffered a brief injury agein 2 few
days after the initial injury and the' wound edges separated a little bit:
He was unaware of any eye problens until he began exercising the horses
again. He noticed this tine that he had to move his head Eron left to
right in order to see F211 on either side _ This loss of side vision has
definitely improved but he still has sone difficulty_ This is especially
noticeable in the left eye Because of his eye complaints, he wns referred
to 2 Dr, Nelson of Corona and following chis to Sr . Milion #, siller of
Onterio. I do not hava Dr . eiller' s reports So I 25l unaware of exactly
what his fincings were, At the pre sent tim? Mir 0 Sirhan' s complaints
prinarily are as fo]lows:
He notices that he has sorz twi tching of the eyelid when he looks
to the lefc_ This involve5 the left eyelid primarily. He also has the
samie type of twitching when he wrinkles his forehead 01 mekes facal
moverents, He has had no subsecuznt unconscious attacks, no dizziness,
no weakness of either arns , hands , legs or feet, He complains of a
persistcnt in the superlor nasal aspect of the left, orbito My
exaiination was 25 follows:
His vision K2s found to be 20/20 iIl cither eye uncorrected He hed no
signi ficant refraceive error, Extrocular @uscles vere found to bc intact ,

the pupils were round , Were regular, and reacted wzll to light and
accomodation. The patient ' 5 eyes Fere dilated and an examination of the
fundus was performed No significant abnorialities Kere found
Examination was perforied both with the direct and indirect ophthal-
Mo scopZ . Slit lamp examination sho no flare or cells in cither eye.
Both lenses are clear 2nd the Iedia eppcared clearo_ Visual field
examination indicates 2 full field with a moderate 2nolint of generel
constriction in the left field, This constriction is inconstant The
patient' s Founds are well healed _ however' there is a persistent tenderness
over the superior orbital ridge medially and chere f5 a Sliz1l emount: of
fullness in this 212a renaining. The tenderne s5 i $ medial to the suprae
orbital dotch and is epparently aggravated whian the patient looks both to
the left and upward to the left There is a fibrotic band extend from
this general area dowiward to the area just below the lower canthal
ligamento He claims that this makes hin hava 2 ratier tight sensation
when he looks to the left. I could demonstrate ro abnormal diplopia,
in fact my findings are remarkably negative with tke exception of the
tenderness and the subcutaneous band shich I nentioned
0 At the sent
time, I do not feel like operating on the area which is described , and
releasing this bznd_ I feel that we should wait for a period of about
one aonth ycte At the time of releasing this subcutaneous band I believe
t would be advi sable to investigate tia original wound 2r2a for the
possibility 0f 2 foreign body reaction giving hitl the persictent pain
which he feels and is described above X-Rays ordered by mle have indicated
no evidence of 2 foreign body , no evidence of any fracture s in and abolt
the orbit, and s2id X-Rays are essentially negative: The X-Rays w2re taken
by Dr . Robert Freeman, of this addie55 I shall see Mr , Sirhan again
repeat visual field tests to maxe sure that tficre is no recurrent
abnoreality. Inasmuch a5 ha was unconsciolis and had rot bzcn seen by 2
neurologist 01 2 nelrosurgeon I believe it would b2 advisable to hava
him seen by 2 neirosucgeon Eo rule out any damage to the brain that might'
have occured et the tinte 0f this injury. I have usually referred IRy
pat ients to a Dr . Robert Fiskin, of 960 E.

September 24, 1966 at the Granja Vista Del Rio

Construction Company, Corona , California
Dr ALBORI advised that SIRHAN complained of
considerable discomfort of the spine and lower back whenever
he moved or bent over , or stood for a long time The
discomfort was greater in the morning, at night and during
cold weather He complained that tha inner skin of h1s left
eye tightens and was uncomfortable, espec1a in cold weather.
1 SIRHAN stated that he felt that the 8car was pulling his
M face off balance
SIRHAN related to Dr ALBORI that he missed two
weeks of work after the accident and upon rc turning to work
he was a88igned to tasks of less reeponsibility, due to his
work related accident He stated that hig employment was
later terminated as a result 0f the above injury, in that he
